As a new PRO Medical PT patient, you may have some questions regarding referrals, insurance, and more. The following information is provided to answer many of the most commonly asked questions.
How do referrals work?
A doctor’s referral is not required by PRO Medical Physical Therapy. Many insurance companies also do not require a referral to cover physical therapy services, but some do. If you are uncertain regarding what your insurance requires, we recommend calling your insurance company, or simply call us, and we will be happy to help you find out.
Can non-members use Physical therapy?
Everyone is welcome for treatment – non-members and members.
What should i expect in my first appointment?
Please plan an hour for each physical therapy appointment. For your first visit, please arrive 15 minutes early for registration and new patient paperwork: Patient Intake Form, Medical History, HIPAA Practices, and the corresponding pain questionnaire for the injury. Please bring your insurance card, photo ID and a referral from your physician (if applicable). We recommend that you wear comfortable, loose clothing such as shorts/sweats, T-shirts, or any athletic attire. There are spacious, fully appointed locker rooms and showers available at all times for your convenience. For a bicycle evaluation, you will need to bring comfortable riding apparel including your shoes and shorts in addition to the bike you ride the most.
What treatment should I expect?
PRO Medical Physical Therapy provides superior treatment for all of our patients by a well-qualified physical therapist. A physical therapist is a musculoskeletal expert who can evaluate your condition and identify the source of your discomfort or dysfunction. Upon evaluation the therapist will work with you to devise a treatment plan to alleviate your symptoms. This treatment plan will be catered to your individual needs and may include any combination of our services. Patients are typically seen 1 - 3 times per week depending upon the type and severity of injury. Our ultimate goal is to eliminate your pain and allow you to fully participate in all of your activities.
What forms do i need to fill out?
All new patients and returning patients who have not been seen for a period of time are required to complete several forms to establish and maintain your medical record as well as provide necessary information for the therapist to provide the proper evaluation and treatment. Please complete the forms prior to being seen by your therapist. For your convenience, you may download the forms and fill them out prior to your appointment to make your check-in process faster.
What about billing?
If you have insurance, PRO Medical Physical Therapy will bill your insurance company directly. Otherwise, you will receive a billing statement twice a month. Bicycling related pain syndromes may be eligible for insurance coverage depending on your individual plan. You will be billed at the time of service for comfort and performance bicycle fitting. Childcare may be arranged in advance with our childcare center in Bellevue at (425) 861-6247
Free parking is available in the Bellevue location’s parking garage, and for the convenience of those with injuries or limited mobility, we have several parking spaces designated for physical therapy near the club entrance. Designated parking is available directly outside the Willows Road front door.
