Nutrition counseling is widely covered by many insurance plans for many health concerns. Prior to coming in for your initial session, please contact your insurance provider and ask the following questions:
What are my nutrition therapy benefits?
Are my benefits limited to a specific diagnosis?
Is there a limit to the number of visits allowed?
Do I need a pre-authorization or a referral from my primary care doctor?
Verification of insurance coverage is the patient’s responsibility. Final determination concerning actual coverage will be made by your insurance carrier after the claim is submitted. When checking with your provider, the CPT code for nutrition counseling appointments is 97802. Please note, we are not a contracted Medicare provider.
Please include insurance information on your Nutrition Consult Form and bring your card to your first session if attending in-person. If you are eligible for insurance billing, we will bill your provider for the session. Payment for non-billable sessions is due at the time of service.
24-hour notice is required to reschedule or cancel an appointment. Late cancellation fee is $60.
