Our Dietitians can conduct sessions via video conferencing. Please alert our scheduling staff that you desire to utilize this service when making your appointment. Note: we encourage all patients to verify telehealth coverage specifically with their insurance carrier prior to scheduling.

Prior to your first visit, please complete and submit the Nutrition Consult Medical Form, HIPAA Notice of Privacy Practices, and PRO Medical Privacy Policy found in the Resources at the bottom of this page and submit forms via email. This will allow us to adequately prepare for your visit and make your session more effective. Once we receive these forms, our administrative team will reach out directly to schedule your appointment.

Nutrition counseling is widely covered by many insurance plans for many health concerns. Prior to coming in for your initial session, please contact your insurance provider and ask the following questions:

  1. What are my nutrition therapy benefits?

  2. Are my benefits limited to a specific diagnosis?

  3. Is there a limit to the number of visits allowed?

  4. Do I need a pre-authorization or a referral from my primary care doctor?

Verification of insurance coverage is the patient’s responsibility. Final determination concerning actual coverage will be made by your insurance carrier after the claim is submitted. When checking with your provider, the CPT code for nutrition counseling appointments is 97802. Please note, we are not a contracted Medicare provider.

Please include insurance information on your Nutrition Consult Form and bring your card to your first session if attending in-person. If you are eligible for insurance billing, we will bill your provider for the session. Payment for non-billable sessions is due at the time of service.

24-hour notice is required to reschedule or cancel an appointment. Late cancellation fee is $60.
